I'm a cardiologist. Everyone is celebrating the AHA's new statement that five cups of coffee a day is safe. Almost nobody is talking about the finding that should change what you do tomorrow morning: Your French press may be raising your cholesterol. And your doctor has almost certainly never asked how you brew. Here's the part that should make you angry. Coffee beans contain oily compounds called diterpenes — cafestol and kahweol. Cafestol is one of the most potent cholesterol-raising substances ever identified in the human diet. It suppresses the liver's regulation of LDL clearance. A paper filter traps nearly all of it. No filter, and it goes straight into your cup. This isn't a hypothesis. It was uncovered because Scandinavian populations drinking boiled coffee had measurably elevated cholesterol — and it fell when they switched to filtered. Studies of unfiltered coffee have shown LDL rises in the range of 8-15 mg/dL in regular drinkers, and more in heavy ones. Think about that. We put people on statins to lower LDL. We agonize over ApoB. And nobody in the exam room asks the one question that could be moving a patient's number every single morning for twenty years. Rank your brew, highest diterpene to lowest: Boiled/Scandinavian and Turkish — worst. No filtration at all. French press — very high. The metal mesh stops grounds, not oils. Moka pot / percolator — high. Espresso — moderate. Concentrated in diterpenes, but you drink a tiny volume. Several shots a day adds up. Cold brew — depends entirely on final filtration. Paper-filtered, low. Paper-filtered drip and pour-over — very low. Instant — very low. If you have elevated LDL or ApoB and you drink unfiltered coffee daily, this is a free intervention sitting on your kitchen counter. Do this: switch to paper-filtered or instant for 6-8 weeks, change nothing else, and retest your lipids. Some people will see a meaningful drop. Some won't. But you'll know — and you'll have gotten there without a prescription. If you love French press, a paper filter placed over the pour, or filtering the finished coffee, gets most of the benefit. Three more things buried in this statement: The AFib reversal has an actual randomized trial behind it. Cardiologists told AFib patients to quit coffee for decades based on folklore. The DECAF trial randomized post-cardioversion patients to daily coffee versus abstinence — the coffee group had about 39% lower recurrence. One nuance: caffeine can still increase PVCs, those extra beats you feel in your chest. Different arrhythmia, different answer. If you feel skipped beats, that's your signal. Decaf keeps much of the benefit. The type 2 diabetes signal — 20-30% lower risk, the strongest finding in the entire literature — shows up with decaf too. So the magic isn't the caffeine. It's the chlorogenic acids and polyphenols. If caffeine wrecks your sleep, decaf is not a consolation prize. Your genes decide your dose. CYP1A2 determines how fast you clear caffeine. Slow metabolizers get the palpitations, the anxiety, the 2am ceiling-staring. Your friend who sleeps after an 8pm espresso isn't tougher than you. He's a fast metabolizer. Neither of you is imagining it. Paper-filtered. Black. Early in the day. Watch your own symptoms. Twenty years of "cut back on the coffee" was the wrong advice, aimed at the wrong variable. The dose was never the question. It was always the filter.
