BONUS THREAD — "What Actually Helps" (the practical payoff) 1/ Ten threads on the science of desire, arousal, and what your erection reveals about your heart. Now the thread everyone's been waiting for: what actually helps. No injections. No internet mystery vials. Just the levers with real evidence behind them — the ones I'd tell my own brother to pull, in the order I'd pull them. 2/ Lever 1: Move your body. This is the most underrated sexual drug on earth. Exercise repairs the endothelium — the vessel lining behind both erections and heart health — directly. It raises nitric oxide, drops blood pressure, improves insulin sensitivity, lifts testosterone and mood. Men who exercise regularly have measurably lower rates of ED. There is no pill that does all of that at once. 3/ Lever 2: Sleep, and get tested for sleep apnea. Most of your testosterone is made during deep sleep. Wreck the sleep, wreck the hormone. And untreated sleep apnea is a quiet destroyer of both erections and hearts — it hammers the vessels all night long. If you snore and you're tired and your libido's gone, this is not a coincidence. Get a sleep study. 4/ Lever 3: Eat like your vessels depend on it — because they do. A Mediterranean-pattern diet is one of the few eating patterns actually shown to improve erectile function and cut cardiac risk. Olive oil, fish, plants, nuts, less processed junk and sugar. Nitric oxide — the exact molecule Viagra's whole mechanism depends on — is built from a well-fed, well-exercised endothelium. Feed the machine. 5/ Lever 4: The supplements with real mechanism (not magic). L-citrulline, 3–6g, raises nitric oxide and mildly improves blood flow — same pathway as the pills, gentler effect. L-arginine works alongside it. These are legitimate, food-adjacent, and low-risk for most people. Skip the "male enhancement" blends. They're mostly caffeine, hope, and sometimes hidden drugs. Mechanism over marketing. 6/ Lever 5: The pills — used intelligently, not secretly. Cialis (tadalafil) is the most useful PDE5 inhibitor for most men because of its long half-life. A daily low dose also treats prostate/urinary symptoms, and there's real interest in what steady vascular support does over time. But here's the rule: a PDE5 pill should be a decision made with a doctor who checked your heart first — not a thing you order around the workup. The pill can mask the warning I spent ten threads teaching you to read. 7/ The honest order of operations: Get evaluated → fix sleep, movement, diet, blood pressure, blood sugar → add the food-based support → then, if needed, the right pill with medical guidance. That sequence treats the whole man. It also happens to protect the exact organ trying to keep you alive. The bedroom and the heart heal on the same to-do list. 8/ Here's why I write all of this, and why it's worth your inbox. The internet will sell you a vial and a dosing protocol and call it optimization. I've spent 25 years watching what that actually does to hearts — and watching what genuine understanding does instead. I write the version your cardiologist would tell you if he had time to sit down. The mechanism, the honesty, the caveats, the whole system — not the hype. 9/ If these ten threads changed how you see your own body, that's the free sample. The deeper work — the science of longevity, the heart, desire, meaning, and how the body and spirit actually connect — lives on my Substack. Longer, quieter, no algorithm deciding what you see. Subscribe if you want the full report, not just the headlines. 10/ One more thing. Everything in these eleven threads was written about men — because that's where the questions came in. But the science of desire, arousal, bonding, and vascular health is every bit as real, as under-explained, and as tied to the heart in women. The kisspeptin trials ran in women too. So did the desire drugs. So does the cardiac warning — women's heart disease is more underdiagnosed, not less. 11/ So I'll ask directly: Women — do you want a version of this written for you? The desire science, the arousal biology, the bonding chemistry, and the cardiac warning signs the medical world keeps missing in you specifically. If enough of you say yes below, that's the next series. Tell me.
