Every time you brainstorm and come up with some CRAZY scenario you then decide how likely it is to happen, how bad the result could be, and then invest the right level in prevention and mitigation. You then verify those assumptions and work with testing. THIS IS THE ACTUAL DEFINITION OF ENGINEERING!
